Occupational Therapy and lactation (CLC) mobile and outpatient visits:

Lactation IBCLC mobile visits:

• Accept most Anthem/ BCBS PPO plans, Cigna PPO plans, and UHC commercial plans (up to 6 visits covered). Please message or call us for benefit verification and authorization of coverage.

Groundworks is actively in process of credentialing with other insurances and managed care agencies.

Not in network?
Groundworks offers competitive private pay rates.

If desired, an itemized invoice (superbill) can be provided to submit to your insurance for reimbursement. A doctor's prescription for OT is highly recommended, as it will help increase the likelihood that insurance will reimburse some or all costs of sessions. 
Some clients are fond of Reimbursify services to simplify the process of submitting out of network invoices, with a higher rate of reimbursement.

It is important to note that all therapy services are not created equal and most have different skill sets and expertise. It is important to find a provider who can best meet your needs, therefore potentially requiring less treatment sessions to achieve your individual goals.

Payment

Private pay and copays due at time of service. Cash, check, credit card, and HSA/FSA cards are accepted.
